Register with our practice

If you would like to register in person at the practice please download the form by clicking this link NHS New Patient Registration Form (PRF1 FORM). You can also register online by using the link here. Please call to book an appointment to register.

You need to bring identification (for example: passport or drivers licence) and proof of address (for example: a bank statement or utility bill issued in the last three months).

Once you are registered you will be invited to attend a new patient health check. So that we update your current medical information.

New born babies with parents already registered with us
We will need a copy of the birth certificate so that we can register your baby. This must be done prior to the babies 6 week check-up.

If you have any further questions about registering, please do not hesitate to contact a member of the reception team who will be able to guide you through this process.

If you would like to register with us you will need to check that you live within our catchment area.

Named GP

All patients at Beaumont Lodge Medical Practice have a named, accountable doctor who is responsible for coordinating their care.

Your named doctor will be allocated to you by the practice. You can still talk to or make appointments to see any of our doctors or nurses, you do not have to see/speak to your named GP.

If you have a preference and would like to request a particular doctor at the practice to be your named GP please talk to one of our receptionists.
